Financial Responsibility

It is expected that deductibles, co-payments, and other amounts not covered by insurance will be paid prior to services being provided. Accepted forms of payment include personal check and credit card. Payment may be made over the phone prior to your procedure or at check in at Tavares Surgery Center. If you have questions at any time, let us know.

Questions About Your Bill

After you’re discharged, a statement will be mailed to you for any balance due after your insurance company has processed your claim. Payment is due upon receipt and may be made by personal check or credit card.

Procedure Charges

We will verify your insurance in order to collect your liability and will do our best to give an accurate estimation based on the information obtained from your insurance company for the services provided and billed by both Tavares Surgery LLC and Rest Assured Anesthesia LLC. However, it is ultimately your responsibility to check with your insurance company so that you are aware of any deductibles, co-pays, or co-insurance that you may owe. Payment for any deductibles, co-pays, or co-insurance are due at the time of service unless prior arrangements have been made with the billing department.

These bills may be from:

Tavares Surgery, LLC (for the facilities, equipment, supplies, staff, etc.)
Rest Assured Anesthesia, LLC (for the administration and supervision of anesthesia, if administered)
Your Physician (professional fee from your physician who performed your procedure)
The pathology laboratory (for specimens collected during your procedure)
